Patient-Centered Medical Home
The Patient-Centered Medical Home," is an opportunity for us to continually improve the care we deliver," says Dr. Gorman. "By going through this NCQA recognition process, it has pushed us to be better doctors and to improve the quality of our patients’ care. This is a way of life for us now and we believe that our patients and our practice are better for it."
We are grateful to the 12,000+ patients we serve. With this important recognition our patient-centered work is not over. We will build on this success and continue to find better ways to care for patients. The PCMH model is multi-faceted. Below are some of the ways we intend to improve the care we deliver:
1. With the use of surveys and the advice and counsel of our Patient Advisory Board, we will continue to invite
input from patients themselves on how we can improve the care we deliver. This has been an important key
to our medical home approach the last couple years.
2. We are working to improve access to care in an effort to provide the services our patients want, when they
want them. The goal is to be able to offer a same-day visit with any provider in the office each day.
3. With the use of disease management and health maintenance tools in our computer system, we are
further developing our methods for providing evidence-based care to patients with chronic conditions such
as diabetes and hypertension, in an effort to truly make a difference in the health and well-being of these patients.
4. We will continue to explore new ways to communicate more effectively and efficiently with our patients.
5. We will move forward with new technologies not for the sake of being “cutting edge” but only if they improve
the care we deliver or provide efficiencies in delivering that care.
Westfield Family Physicians, PC Receives Important National Recognition
by the National Committee for Quality Assurance (NCQA)
March 2011 - At Westfield Family Physicians we have always tried to be a patient-centered practice.
Without careful attention to our patients’ needs, we would not be carrying out our Mission Statement.
For this reason, when we were approached in 2008 by Independent Health, a health plan based in
Buffalo, NY, to participate in a pilot project for a Patient-Centered Medical Home (PCMH), we gladly
accepted the challenge, along with 17 other practices in Western, NY. We committed to a two year

“The patient-centered medical home promises to improve health and health care,” said NCQA President Margaret E. O’Kane. “The active, ongoing relationship between a patient and a physician in medical homes fosters an all-too-rare goal in care: staying healthy and preventing illness in the first place. PPC-PCMH Recognition shows that Westfield Family Physicians has tools, systems and resources to provide their patients with the right care at the right time.”

and have found the relationship we have developed with Independent Health to be very valuable. Patient-centric models are taking
hold in communities across the country to achieve what healthcare reform movement refers to as the Triple Aim:
(1) Improve the health of the population,
(2) enhance patient experience of care with better quality, access, and reliability, and
(3) control healthcare costs.
In September 2010 our main location in Westfield, NY was awarded the highest level (level 3) of recognition for Physician Practice Connections® - Patient-Centered Medical Home™ (PPC®-- PCMH™) by the National Committee for Quality Assurance (NCQA). In January 2011, our satellite location in Sherman, NY was awarded the same recognition. NCQA is a private, non-profit organization dedicated to improving health care quality, and recognizes the vital role clinicians play in advancing quality.
