QUALITY ASSURANCE INITIATIVES IN 2006

The healthcare providers at Westfield Family Physicians are involved in various quality assurance programs in 2006. These programs are on the cutting edge of health care measures today and will assist the providers in achieving optimal health outcomes for their patients.

Some of the ongoing quality assurance programs deal with the management of cardiovascular disease, hyperlipidemia and diabetes.  Following national standards of care for these diagnoses, measures are taken to promote the patient’s compliance with the plan to care for the diagnosed disease(s).  Measures take the form of lab work, vision screening, exams etc. 

It takes cooperation between the healthcare provider and the patient to achieve preset goals for better health when dealing with these chronic diseases.  It is only after the healthcare provider reviews the patient’s results that (s)he is better able to make more precise recommendations to improve the patient’s health.  Practitioners know what needs to be done to provide good care for chronic conditions, but it takes the provider and patient working together to achieve goals in these measures.

The goal of all quality assurance programs of WFP is to achieve better outcomes when dealing with chronic diseases.  WFP wants each patient with a chronic disease to live a long, happy, and healthy life.  We want to work with you.
